When to create an ADR

• A decision changes the Technology Stack or key behavior from the spec
• An implementation reveals a discrepancy requiring a spec update

Steps

1. Run author-adr prompt
   • Context: discrepancy or decision
   • Includes impacted Spec IDs, Plan Tasks, Work Items
2. Review and accept the ADR
3. Run propagate-adr prompt
   • Updates spec, plans, breakdowns, and docs with consistent changes
